Childcare
Enrolment
Registration is on a first come, first serve basis. Although priority is given to siblings, we cannot guarantee a spot.
We offer both full and part-time childcare. In the event a full-time space is open, it will be made available for part-time attendance on a temporary basis until it can be filled full-time.
Parents who have their children enrolled on a part-time temporary basis will be given priority with the option of enrolling their child full-time.
*Please note: Two children attending part-time on alternating days are permitted to occupy 1 full-time space.
The following forms are required by Education and Early Childhood Development to be completed prior to your child’s first day;
1. Parents signed statement of service
2. Child profile
3. Proof of immunization or form to excuse
4. Permission for administration of medication in certain circumstances
5. Permission for administration of acetaminophen
6. Permission for emergency care and transportation
7. Permission for activities off the premises
8. Permission for application of lotions in the form of diaper cream, sunscreen or bug spray
9. Permission to leave the facility with persons authorized by parent or guardian
10. Permission to post photographs of your child at our facility to illustrate learning
11. Permission, in the case of illness or soiled clothing, your child to shower or bathe, or showering or bathing your child
12. Permission for extended nap past 2 hours when applicable
You are encouraged to bring your child for two 2 hour visits, free of charge, prior to your child’s first day to adjust your child to the day care. This way he/she can get to know us and the other children and recognize this as a safe and welcome environment.
Forest Park Childcare requires a one-time, non-refundable registration fee of $30.00. In addition we require a deposit of one week’s payment that will be applied to your first week of enrolment. Both fees are due at the time of acceptance. Enrolment will only be guaranteed once these fees are paid.
In the event of parental separation and/or where visitation restrictions are in place, the custodial parent must notify us of the custody and access arrangements. A copy of the custody agreement or court orders, if applicable, should be provided for the child’s records.
Discharge
Thirty days written and signed notice is mandatory when removing your child from care or any requested changes to the registration are required. If notice cannot be given, the payment for thirty days is mandatory. This allows us time to fill the opening.
If you remove your child for any reason other than vacation time or an illness, there is no guarantee that the position will be available if you wish to return.
Confidentiality
All children and their personal records are regarded as confidential. Written consent of all those involved, including a child’s parent is needed before releasing private and personal identifying information relating to an employee, a child attending the facility, or their family.
All records are made available to authorized Departmental staff, upon their request.

Please label all of your children’s belongings with first and last initials.
Personal Belongings
Each child has their own box to keep personal belongings in. Children’s personal belongings are welcome as long as they do not propose any risk to themselves and others. All personal belongings must be labeled with the child’s first and last initial. To help prevent the spread of communicable disease and skin infections, we ask that personal belongings that are not toys are not shared.
Children will not be required to share comfort objects, but if they bring a toy from home they are encouraged to share it at some point during the day. We take no responsibility for lost or broken toys brought from your home. Please ensure any items coming from home are age appropriate.
Please ensure proper seasonal attire is sent. This includes splash pants, rubber boots and a hat in spring/summer, and mittens, snow boots, a hat and snowsuit in winter.
It is your responsibility to provide diapers and wipes. For your convenience, a supply may be left at our home and we will alert you when it is low.
Please do not send candy or gum for your child, as chewing it can be a safety risk.
